Case managers are used throughout the healthcare system to help people identify specific issues in their healthcare, and to provide a step-by-step process of how a person should proceed to get help. Steps of Case Management
- Intervention Facilitation: If someone contacts Shadow Mountain Recovery about getting a loved one into treatment, we try to set them up with a case manager who can help them with the very first step: intervention. Unless your loved one has already agreed to undergo treatment, then an intervention is necessary to powerfully communicate the harm that their actions are causing you. A case manager helps plan an intervention, and can provide a mediator to help effectively communicate your case to your loved one.
- Detoxification Coordination: If a person’s addiction is physically severe, then it is likely that they will need to undergo detoxification before they can be admitted into an inpatient recovery program. A case manager will either get you set up at one of our centers for detoxification, or can recommend a trusted detox center near your area.
- Mental Health Therapy Planning: We believe that the best options for addiction recovery utilize effective therapeutic methods. However, different clinical modalities may be better for different clients. A case manager can help you figure out what types of therapy are necessary for you, and whether you need individual, group, or family therapy, or even a mixture of all three.
- Financial Planning: Insurance and addiction treatment can be a complicated combination, especially on top of an already stressful situation. A case manager can help you figure out the best payment method for treatment, and can assist with the communication with your insurance company, in order to reduce the financial burden.
- Aftercare Monitoring: After a patient finishes with inpatient or outpatient treatment, they usually undergo a period of aftercare that includes support groups, periodic therapy sessions, and potential medications. A case manager helps facilitate the aftercare process, and can set up appointments to ensure that a patient is maintaining their sobriety.
